Ivermectin has been given billions of times in mass drug administration programmes over four decades, which gives it an unusually large real-world safety record. At standard doses it is well tolerated. The risks that do exist are worth understanding precisely rather than vaguely — particularly the dose-related neurological ones and the reactions that come from parasite death rather than the drug itself.
The effects reported most often are mild and settle without intervention: dizziness, nausea, diarrhoea, abdominal discomfort, headache, drowsiness, and a transient rash or itch. Taking the dose with food reduces gastrointestinal upset for many people, though it also increases absorption.
When ivermectin kills a heavy parasite burden, the dying organisms release antigen and the immune response to that produces fever, itching, rash, swollen and tender lymph nodes, joint pain and sometimes low blood pressure. This is the Mazzotti reaction, and its severity tracks parasite load rather than drug dose.
It is important to recognise because it is frequently mistaken for a drug allergy. It is most prominent in onchocerciasis and is a sign the drug is working.
Ivermectin's safety in mammals rests on P-glycoprotein at the blood-brain barrier actively pumping the drug out of the central nervous system. Serious neurotoxicity — confusion, ataxia, seizures, reduced consciousness — occurs when that protection is overwhelmed or defective.
That happens in three situations: very high doses, co-administration of drugs that inhibit P-glycoprotein, and rare individuals with impaired barrier function. It is also the mechanism behind the well-documented sensitivity in certain dog breeds carrying the MDR1 mutation. At standard human doses with an intact barrier, serious neurological events are rare.
Most serious human ivermectin poisoning reported to poison control centres has involved veterinary formulations — particularly horse paste, which is concentrated for animals weighing several hundred kilograms. Symptoms of overdose include vomiting, severe dizziness, ataxia, confusion, hypotension, seizures and, in severe cases, coma.
Veterinary products also contain excipients not evaluated for human use, and are not manufactured to human pharmaceutical standards. Everything sold here is human formulation.
Ivermectin can potentiate warfarin, and INR should be monitored if the two are combined. Caution is warranted with sedatives and with drugs that inhibit P-glycoprotein. Alcohol may increase absorption and compound dizziness.
In pregnancy, data are limited and ivermectin is generally deferred where treatment can safely wait. It passes into breast milk in small amounts. In children, it has an established record in mass administration programmes but oral use has typically been limited to those above 15 kg. Liver or kidney impairment warrants clinical supervision. Any of these situations should be discussed with a clinician.
