Ivermectin is used in both human and veterinary medicine, and the active molecule is the same. The formulations are not. The differences in concentration, excipients and manufacturing standards are large enough to matter clinically, and they are the reason most serious human ivermectin poisonings involve animal products.
A human ivermectin tablet contains a precisely measured quantity — typically 3 mg, 6 mg or 12 mg. A tube of equine ivermectin paste is formulated for a horse weighing 500 to 600 kilograms and contains a correspondingly enormous total dose in a form designed to be squeezed out by eye against a weight scale on the barrel.
Dividing that accurately for a human is not realistically achievable. Small errors in a viscous paste translate into very large dose errors.
Veterinary formulations contain carriers, thickeners, preservatives and flavourings selected for animals and never evaluated for human ingestion. Pour-on and injectable products contain solvent systems that can be harmful if swallowed. The active ingredient is only part of what you are taking.
Human pharmaceutical manufacturing operates under GMP requirements covering content uniformity, impurity limits, dissolution testing, stability and batch documentation. Veterinary manufacturing standards exist but differ, and the tolerances are set for animal use.
US poison control centres recorded a sharp rise in ivermectin exposure calls during periods of heightened public interest, and analyses of those cases found that the more severe outcomes clustered in people who had taken veterinary formulations rather than human products. The pattern is consistent: the harm tracks the formulation.
