There is no documented direct pharmacological interaction between ivermectin and ethanol. Alcohol is not listed as a contraindication in the FDA labelling, and no clinical study has reported a specific adverse interaction. That said, there are three sensible reasons to be cautious, and they are worth understanding rather than dismissing.
Ivermectin's most common adverse effects are dizziness, drowsiness, nausea and, less often, transient visual disturbance. Alcohol produces the same effects through an unrelated mechanism.
Taking both together does not create a new toxicity, but the effects stack. Someone who would have felt mildly light-headed on ivermectin alone may feel markedly unsteady after a few drinks. This matters most for driving and for anyone already prone to dizziness.
Ivermectin is cleared almost entirely by the liver, principally through CYP3A4 metabolism, with elimination in bile and faeces. Ethanol is also metabolised hepatically and, in sustained or heavy use, alters the expression of several CYP enzymes.
For an otherwise healthy person having a drink or two, this is not a meaningful concern. For someone with existing liver disease, hepatitis, or a heavy chronic drinking pattern, impaired hepatic clearance can prolong ivermectin exposure and raise the chance of neurological side effects. In that situation ivermectin should be used under medical supervision regardless of alcohol.
When ivermectin is given for a heavy parasite burden, dying parasites can release antigenic material that produces fever, aching, headache and malaise — a Mazzotti-type reaction in onchocerciasis, or simply a temporary flare of itch in scabies.
Alcohol makes these symptoms harder to interpret. If you cannot tell whether you feel unwell because of the treatment or because of what you drank, you lose useful clinical information at the point where it matters most.
Most clinicians who are asked suggest avoiding alcohol on the day of dosing and for 24 to 48 hours afterwards. Plasma levels peak around four hours and fall substantially over the following day or two, so this window covers the period of highest exposure and highest chance of overlapping side effects.
For a multi-dose course such as scabies treatment, the doses are typically 7 to 14 days apart, so this need not mean abstaining for the whole course — only around each dose.
