Two drugs are frequently discussed as treatment options: metronidazole, the long-standing standard, and albendazole, a drug originally developed for worms that has also shown activity against this parasite. It lacks the alcohol interaction and metallic-taste problem. The concerns that do exist relate mainly to longer courses used for other conditions — weeks of treatment for hydatid disease have been associated with reversible elevations in liver enzymes and, rarely, bone marrow suppression, effects far less relevant at the short doses used for giardiasis but still a reason clinicians sometimes check liver function with repeated courses.
- Pregnancy: Albendazole is contraindicated in the first trimester and used with caution later in pregnancy, based on teratogenicity and embryotoxicity observed in animal studies. Metronidazole's pregnancy data are more reassuring for later pregnancy, though it is generally avoided in the first trimester absent a compelling reason. Neither decision should be made without direct medical guidance, since the specific gestational timing changes the risk calculus considerably.

Metronidazole for giardiasis is typically dosed two to three times daily for five to seven days. Albendazole's studied regimen for this indication is usually a single daily dose for five days. In the United States, metronidazole is FDA-approved for amebiasis, trichomoniasis, and various anaerobic bacterial infections; its use for giardiasis, while it is the regimen most clinical guidelines recommend and is considered standard of care, is technically an off-label use with respect to the FDA label itself. Albendazole's FDA approval covers hydatid disease and neurocysticercosis; its use for giardiasis is also off-label in the United States, supported by the international trial literature discussed above rather than by an FDA-reviewed indication specific to this parasite. Tinidazole, a related nitroimidazole, does carry FDA approval specifically for giardiasis and is worth knowing about as a third option, though a full comparison of tinidazole is beyond the scope of this article. None of this means off-label use is inappropriate — off-label prescribing based on solid trial evidence is common and often entirely reasonable — but patients deserve to know when a drug is being used outside its formal approved indication so they can ask informed questions.
