By placing an order on Ivermectin.com you agree to these terms. They repeat, in one place, what you confirm on the order page.
1. What you certify when you order
- I understand these statements have not been evaluated by the FDA. These products are not intended to diagnose, treat, cure, or prevent any disease.
- I certify that the patient is NOT currently taking any of the following medications: Warfarin (Coumadin/Jantoven), Sirolimus (Rapamune), Tacrolimus (Advagraf XL/Envarsus XR), Erdafitinib (Balversa), Lasmiditan (Reyvow), Tepotinib (Tepmetko), Erythromycin, Itraconazole, Ketoconazole, Rifampin (Rifadin), or Verapamil.
- I and the patient understand that we are choosing to use these medications at our own risk. We have reviewed the medication guide and acknowledge common side effects including: dizziness, pruritus, nausea, diarrhea, joint pain, muscle stiffness, swollen lymph nodes, and low blood pressure.
- I certify that patient does NOT have Severe Asthma, Severe Liver Disease, hyperactive Onchodermatitis, Concurrent Loiasis or Onchocerciasis.
- I certify that the patient is over the age of 18 and not pregnant, nor will be breastfeeding while taking any of these meds.
- I certify that I will do my own research and consult a pharmacist or physician on proper dosages for my body weight.
- I certify that patient is NOT allergic to any of these meds.
- I understand that all medications ship from our USA facility.
- I confirm that this order will be shipping to a USA destination.
2. Paying, and the name on your statement
Our payment partners bill under their own names, not “Ivermectin.com”. Under the pay button of the method you choose, you agree to:
- Card: I also agree to the Terms of Service. This charge will appear on my card statement as "BALANCE SERVICE or WEB BALANCE or AMBER BALANCE" (or the processor name on my confirmation page), not Ivermectin.com. After my payment goes through, I will upload a screenshot of this charge line from my bank or card statement using the secure upload link Ivermectin.com emails me (ivermectin.com/proof.php).
- Card through a payment app (Crypto.com Pay, Topper, Stripe / Link, MoonPay, Transak): I also agree to the Terms of Service. This payment will appear on my statement as "UPHOLD", not Ivermectin.com. After my payment goes through, I will upload a screenshot of this charge line from my bank or card statement using the secure upload link Ivermectin.com emails me (ivermectin.com/proof.php).
- Pay by Bank / eCheck: I also agree to the Terms of Service. This payment will appear on my bank statement under the payee name shown on my order confirmation, not Ivermectin.com. After my payment goes through, I will upload a screenshot of this charge line from my bank statement using the secure upload link Ivermectin.com emails me (ivermectin.com/proof.php).
- Melio: I also agree to the Terms of Service. This payment will appear on my statement as "GermProof or OCW - GP", not Ivermectin.com. After my payment goes through, I will upload a screenshot of this charge line from my bank or card statement using the secure upload link Ivermectin.com emails me (ivermectin.com/proof.php).
- Wire, check, cash, bill pay, e-Transfer, XRP: I also agree to the Terms of Service. After I send my payment, I will upload a screenshot or photo of the payment record (my statement line, wallet transaction or receipt) using the secure upload link Ivermectin.com emails me (ivermectin.com/proof.php).
Card totals include the processing fee shown before you pay and are charged in US dollars.
3. Why we ask for a statement screenshot
Because the charge carries our processor’s name, banks sometimes do not recognize it. A screenshot of the charge line on your statement, uploaded through your secure link (ivermectin.com/proof.php), shows the payment is yours and recognized. It protects you and us if a charge is ever questioned, and it keeps card payments available to everyone. If anything on your statement looks wrong, email [email protected] first.
